I am a senior in need of services.
Care Management is a program that provides support and services to frail and disabled seniors and adults. The goal is to put services in place to enable seniors to live as independently as possible, avoiding inappropriate institutionalization. This includes services for caregivers as well as for senior (ages 60 +) caregivers who are caring for a minor child or children. These services include client intakes and assessments, referrals to needed services which may include medical evaluation, legal assistance, behavior management, transportation and in-home assistance, as well as respite, supportive counseling and support groups for caregivers. I am homebound and would like to have meals delivered to my home.
We serve Home Delivered Meals (meals-on-wheels) to seniors who are homebound because of illness, age or frailty. These are the same meals we serve in our dining room and we serve them the same five days per week. We have a frozen home delivered meal program as well and can often provide meals for weekends.

The Home Delivered Meals Program is a temporary service designed to help you through a period where you can’t get out, cook or shop for yourself. A care manager will make a determination if you qualify. There are services available if you are homebound for a longer period of time.

I am unable to get around and need a ride
The Inglewood Senior Center offers several transportation opportunities to residents of Inglewood Lennox and Hawthorne. Our services include: para-transit (wheelchair accessible vans), taxi coupons, I-line shuttle and discount bus passes. We also refer our clients to other modes of transportation. I have a family member with Alzheimer’s and I need help.
The Care Advocate project is an integral component in the ongoing care management services available through the Senior Center. Through a grant from the Alzheimer's Disease and Related Disorders Association, Inc., a Care Advocate is available to assist clients with Alzheimer's Disease and other dementias, their caregivers and families.

These services include client intakes and assessments, referrals to needed services which may include medical evaluation, legal assistance, behavior management, transportation and appropriate out-of-home placement, as well as respite, supportive counseling and support groups for caregivers.

The Care Advocate identifies and accesses the informal neighborhood networks and conducts community education presentations. For more information, call (310) 412-4380.

I need help with long term care.
The Information and Assistance desk at the Senior Center is the place to go for all sorts of information relevant to senior citizens and disabled adults. The staff and volunteers will assist you in obtaining the services you may need, provide referrals and answer your questions. The office is open from 8 a.m. to 5 p.m., Monday through Friday.

You can get assistance with food, health services and transportation; Social Security and Medi-Cal; rebates and discounts, educational opportunities, and much more. If they don’t have the answer, they will assist you in finding it. For more information, call (310) 412-4370.

I need to know about Medicaid or Medicare.
